Flu Shots at Riverside

Urgent Care will be at Riverside Academy to give Flu Shots on October 12th from 8am-Noon. All students and staff are encouraged to participate. Any students under the age of 18 must be accompanied by an adult. Please see the flyer for details and fill out the form below. Form must be returned to school by FRIDAY, OCTOBER 6TH.
